Case Management Services

The MCBHC’s Case Management Department provides needs assessments, service planning, linkage to community supports and resources, and service coordination and monitoring for individuals. Service planning builds on the strengths of the individual with the overall goal of improving the client’s independence in daily living. Case Management services are voluntary. The MCBHC offers the following array of case management services:

  • Blended Case Management, and Administrative Case Management;
  • Peer Specialist
  • Drug and Alcohol Utilization Review and Drug and Alcohol Case Coordination;
  • Intellectual Disabilities Home and Community-based Waiver Case Management, Targeted Service Management;
  • Early Intervention Service Coordination
