Case Manager III - CASSP
This position will serve as the Coordinator of a system to provide coordinated and collaborative care to address the needs of children, adolescents and families in Mercer County experiencing emotional and behavioral issues and involved in multiple service systems. This will entail the development and implementation of a coordinated service approach and programs involving entities such as Children & Youth, Juvenile Probation, the area school districts and affiliated entities and the D&A and MH/MR components of the Behavioral Health Commission. The CASSP Coordinator will also lead the facilitation of an internal process to afford inter-agency and intra-agency treatment team service planning and coordination. Position will promote and ensure that CASSP principles are applied across the Mercer County and its child/family service system.
Work will be performed with considerable discretion to the related duties of the position with direct accountability to the children's supervisor. General guidance will be provided by the CASSP Steering Committee. Position will contribute to the development of policies and procedures, provide trainings and education both internally and externally. Position must maintain strict confidentiality requirements and will enforce such requirements within responsible unit. Position will also participate in organization quality assurance activities, as well as be indoctrinated into the total quality management philosophy. Position will participate in plan development as requested by the CEO of the BHC.
Qualified candidate must have at minimum a bachelor's degree with a minimum of three years experience in Human Services. Masters degree preferred. Act 33 and Act 34 Clearances will be required. Competitive salary provided, commensurate with experience and education of the candidate.
