MCBHC's Mental Health Case Management includes three levels of services
Intensive Case Management (ICM) provides assistance to adults with serious and persistent mental illness and children at risk of serious mental illness through intensive contact and a strengths-based approach; and by accessing, coordinating, and monitoring needed resources and services. The primary focus of ICM services includes increasing the self-sufficiency and independence of living for the consumer, promoting social supports, increasing the consumer's ability to maintain treatment, and decreasing the need for inpatient hospitalization.
Resource Coordination (RC) provides assistance to adults and children with serious mental illness who do not require the intensity and frequency of contact provided through ICM. RC services utilize a comprehensive strength/needs assessment an service planning process to assist consumers in achieving increased independence, improved quality of life, and stability of symptoms.
Administrative Case Management (ACM) provides assistance to adults and children with any mental health diagnosis that do not meet eligibility criteria for either ICM or RC services, but do present a need for case management support. ACM services are provided at a less intensive level than ICM or RC. Consumers who are initially entering the Mental Health system and do not have a significant treatment/hospitalization history are often targeted for ACM services.
Other Programs
Community Hospitalization Integration Project Program (CHIPPS) is provided to a specific population of Mental Health consumers that have extensive treatment histories at the state hospital level, primarily Warren State Hospital. These individuals are diagnosed with Serious Mental Illness (SMI) and receive enhanced supports within the community, including case management, residential, supportive housing, and social/vocational rehabilitation. Additional Psychiatric Nursing services are provided by the MCBHC to these individuals to address their psychiatric needs and provide monitoring and education in areas such as medication compliance and mental health symptom reduction. CHIPP services also benefit individuals that are in need of enhanced mental health support including those that are diverted from state hospital admission and/or utilize multiple community mental health services.
