Drug and Alcohol Administrative Care Management
D&A ACM provides funding resources and the monitoring of drug and alcohol treatment for individuals in need of detoxification, residential rehabilitation, halfway house, and outpatient services. The primary role of D/A ACM is to assure that clients are receiving the level of care necessary to address their substance abuse needs and that they meet eligibility for funding through the appropriate eligibility guidelines established.
Drug and Alcohol Intensive Case Management
D/A ICM assists adults and adolescents with drug and alcohol abuse or dependency in identifying specific needs for services. D/A ICM services assist clients in setting goals to address obstacles that may be hindering their ability to successfully complete treatment and maintain their sobriety. D/A ICM does not replace substance abuse treatment but is designed to provide additional support to enhance the recovery process.
Consumer needs may include housing, employment, education, medical, childcare, transportation, clinical treatment, and social support systems. Pregnant women, women with children, IV drug users, and adolescents are considered target or priority population groups for D/A ICM services.
Forensic Case Management
This program provides evaluation, screening, treatment recommendations/referral and service coordination to inmates in the Mercer County Jail with identified mental health and/or substance abuse issues. The Forensic Case Manager coordinates services closely with the Forensic Therapist position in the jail. The Forensic Case Manager serves as the primary liaison to the Mercer County Court of Common Pleas to ensure that inmates are properly identified, evaluated, and established with an appropriate level of treatment.
