Case Manager II - Intake
Position will be responsible for providing intake services to individuals with behavioral health needs.  Intake worker will assist consumers in accessing appropriate services by determining their need for treatment and referring to recommended levels of care.  Position will provide follow-up data and tracking information related to the screening and referral of clients to services within the established behavioral health service system.  These case management services are delivered within appropriate environments including, but not limited to the Central Intake Unit, school, treatment setting, hospital and home.  Position will complete phone intakes with adults, children, and adolescents. This position will assist consumers during crisis situations and will participate in the agency's crisis intervention services, including daytime and after-hours crisis and delegate responsibilities.

Qualified candidate must have at minimum a bachelor's degree in a behavioral health or other human service related field.  Act 33 and Act 34 Clearances will be required.  Competitive salary with excellent fringe benefits provided, commensurate with experience and education of the candidate.
