MR Supports Coordinator
This position is responsible for providing case management and advocacy services to a targeted population of individuals with mental retardation needs. Supports Coordinator will assist consumers in accessing, linking, coordinating, and monitoring appropriate resources to establish their independence and well-being within the community setting. These case management services are delivered within appropriate environments including, but not limited to, home, community, educational, vocational, and treatment, utilizing a strengths-based approach.
Qualified candidates must possess a minimum bachelor's degree in a behavioral health or other human service related field. Act 33 and Act 34 Clearances will be required. Competitive salary with excellent fringe benefits provided, commensurate with experience and education of the candidate.
