Case Manager
Case Manager position responsibilities include providing case management and advocacy services to a targeted population of adults and/or children in need of behavioral health, mental retardation and/or substance abuse services. This position will assist consumers in accessing, linking, coordinating, and monitoring appropriate resources to meet their treatment needs, improve their level of functioning, and well-being within the community setting. Case management services are delivered within appropriate environments including but not limited to home, community, educational, vocational, and treatment, utilizing a strengths-based approach. Position requires participation in the agency's after-hours crisis on-call services upon completion of mandatory trainings and in-services.
Qualified candidate must have at minimum a bachelor's degree in a behavioral health or other human service related field. Act 33 and Act 34 Clearances will be required. Competitive salary with excellent fringe benefits provided, commensurate with experience and education of the candidate.
