Drug and Alcohol Case Coordination

Case Coordination is an individualized, client-care service designed for individuals with substance use disorders who have non-treatment related needs. The purpose of Case Coordination is to provide assistance in accessing needed services and resources, as well as support and advocacy.

Activities include assessment/evaluation of the client’s strengths and challenges, linkage/referral to community services and supports, service planning and goal setting as well as assisting the client to develop a positive support network.

Case Coordination may be conducted as an office-based or community service to meet the needs of each individual.
