Mental Health Case Management

Mercer County Behavioral Health Commission provides Mental Health Blended Case Management (BCM) traditional Mental Health Intensive Case Management and Resource Coordination Programs incorporated into one program. Specific eligibility criteria must be established before BCM services can begin. The service is available for adults with mental health issues, children/adolescents with mental health issues, or children/adolescents at serious risk of mental illness. This blended model allows for consumers to keep and maintain the same case manager throughout their participation in the program. 

BCM utilizes a comprehensive strength-based service planning process that is consumer driven. Blended Case Managers assist individuals in linking to needed services and supports that will help them meet their goals and objectives. Blended Case Managers further monitor and coordinate service delivery with their consumers. 

All consumers that participate in BCM services have access to a 24/7 case manger on call line that is staffed exclusively by Blended Case Management Staff.
