Drug and Alcohol Case Coordination

Case Coordination is an individualized, client-care service designed for individuals with substance use disorders who have non treatment related needs. The purpose of Case Coordination is to provide assistance in accessing needed services and resources, as well as support and advocacy.

Activities include assessment/evaluation of the client’s strengths and challenges, linkage/referral to community services and supports, service planning and goal setting as well as assisting the client to develop a positive support network.

Case Coordination may be conducted as an office based or community service to meet the needs of each individual.
