Case Coordination is an individualized, client-care service designed for individuals with addictive disorders who have ancillary needs. The purpose of Case Coordination is to provide assistance in accessing needed services and resources, as well as support and advocacy. Activities include assessment/evaluation of the client’s strengths and challenges, linking/referral to community services and supports and, when appropriate; service planning and goal setting, assisting the client to develop a positive support network, advocating if services and resources are not readily available, and following up to ensure the client’s needs were adequately addressed.
